PATIENT INFORMATION
WHAT TO EXPECT FOR YOUR FIRST VISIT
For your first visit at Primary Care Clinic of North Texas, please arrive at least 10 minutes before your appointment time so that you can fill out the new patient paperwork and be checked in by our staff.
We strive to see our patients promptly at their scheduled appointment time. Please assist us in keeping our schedule running on time by following these simple guidelines:
When making your appointment, clearly state the main reason (Annual/Physical, Pap Smear, Sick, Follow up, Medication refill) for your visit to help us schedule the appropriate amount of time with your provider
The provider will only be able to see you for the main reason scheduled that day because add-on issues take time away from other patients who are scheduled that day.
If you are unable to keep your appointment, please call our office at least 24 hours in advance. Cancellations made in less than 24 hours may incur a $20 fee.
WHAT TO BRING TO YOUR APPOINTMENT
Please bring the following for each visit:
Photo ID
Payment via credit card or cash. If you are paying by cash, please bring some change.
A list of all medications you are currently taking or have taken recently, including any over-the-counter medications, vitamins or supplements. Include the strength and dosage of each medication.
Any medical records ordered by providers outside of Primary Care Clinic of North Texas such as your last mammogram, colonoscopy, lab work, X-rays or any other tests performed.
